[PATCH] backend conditional requests 6th release

Hello all,

This is the 6th version of the patch for backend conditional requests
(usually If-Modified-Since). It applies to;

After discussion on varnish-dev, we've changed the meaning of the keep
interval back to what it had been in earlier versions: both keep and
grace are counted down simultaneously after TTL elapses, an object is
ready for eviction after ttl + max(grace, keep), and default_keep ==
default_grace. This has been reflected in the Wiki page:
